These are the specific contest rules for the “Mix Advisory Panel Online” on KUDD-FM. These rules are in addition to the General Contest Rules of KUDD-FM. Complete copies of all contest rules are available at Broadway Media, 50 W. 300 S. #200, Salt Lake City, Utah, 84101, from 9 am to 5 pm, Monday through Friday. To participate in the “Mix Advisory Panel Online” promotion, participants must complete our Mix Advisory Panel Survey online at www.mix1051utah.com during the survey period of 06/26/2018 – 12/31/2018. Participants who fully complete the survey will be entered to win a $250 Target Gift Card. The survey must be completely filled out for entry to win the $250 Target Gift Card. Incomplete surveys will be disqualified from the entry contest. After completing the survey, participant’s info will be entered to win the $250 Target Gift Card. Qualified participants will be randomly selected as a winner for the $250 Target Gift Card. Set date and time of winner selection has not be set, this will be at random. Prize: (1) $250 Target Gift Card. Qualifying participants will be notified via phone or email if they won the Lagoon ticket prize. Tickets must be picked up by winner at Broadway Media (50 West Broadway Suite 200, Salt Lake City, UT 84101) within 30 days of being notified that they are available for pickup.
